St. Jude Medical will provide a Returned Products Kit to organizations and physicians who have explanted devices from patients. This kit contains the following items:
- Postage-paid explant box with shipping label
- Removed Device Information form
- Biohazard bag
- Biohazard labels to seal the explant boxes
